Registration
Patients may register at reception. The following should be noted:
- Although you will be registering with the practice as awhole, normally you will be able to see the doctor or nurseof your choice.
- If you need to be seen at short notice the doctor or nurse ofyour choice may not be available, in which case you maysee someone else.
- When you register at the practice please bring your medicalcard with you together with full details of your previoussurgery and GP.
- Please tell us if you change your name or address.
- Please register your new baby as soon as you can after the birth.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
