Registration
Patients may register at reception. The following should be noted:
- Although you will be registering with the practice as awhole, normally you will be able to see the doctor or nurseof your choice.
- If you need to be seen at short notice the doctor or nurse ofyour choice may not be available, in which case you maysee someone else.

When you register at the practice please bring full details of your previous surgery and GP. All patients will be invited to have a short registration medical with one of our Health Care Assistant. This enables us to record health information about you that might be relevant to the resolution of any adverse symptoms you might have in the future.

Please tell us if you change your name or address.
- Please register your new baby as soon as you can after the birth.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
Registration Form - Please note that it is necessary to complete a short NHS form also when attending the surgery for the first time. We are not able to make this available for download.
